These terms apply to the Complimentary Smile Makeover Assessment Consultation and the 5% Loyalty Benefit available to eligible existing Maxicare Dental patients.
This offer is designed to help existing patients understand treatment options that may be suitable for their oral health. It does not guarantee that treatment will be recommended or that a specific result will be achieved.
1. Complimentary Smile Makeover Assessment Consultation
Who is eligible?
This offer is available to existing Maxicare Dental patients only.
An existing patient means someone who has previously attended Maxicare Dental and has an active patient record with the practice.
What is included?
The complimentary consultation includes:
A Smile Makeover assessment consultation
A discussion about your concerns and treatment goals
A clinical assessment by the dentist
An explanation of treatment options that may be suitable
General information about costs, risks, benefits and alternatives
What is not included?
The complimentary consultation does not include diagnostic services or treatment.
Additional fees may apply if the dentist recommends diagnostic records or further assessment, such as:
X-rays
Digital scans
Clinical photographs
Other tests or records required for treatment planning
Any additional costs will be explained before proceeding.
Is treatment guaranteed?
No. Treatment is not guaranteed.
A consultation is required to assess your oral health and determine whether any treatment is clinically suitable. The dentist may recommend no treatment, further assessment, or alternative treatment options depending on your individual needs.
All dental procedures carry risks and benefits. Individual outcomes vary.

Offer is not transferable

2. 5% Loyalty Benefit
Who is eligible?
The 5% Loyalty Benefit is available to existing Maxicare Dental patients only.
It only applies if the dentist confirms that Smile Makeover treatment is suitable for your oral health and you choose to proceed.
What does the loyalty benefit apply to?
The 5% Loyalty Benefit applies to eligible Smile Makeover treatment fees provided at Maxicare Dental.
Eligible treatment may include treatment options discussed during your consultation, depending on your oral health, clinical suitability and treatment plan.
What is excluded?
The 5% Loyalty Benefit does not apply to:
Consultation fees
Examination fees
Diagnostic services, including X-rays, scans or photographs
Hygiene appointments
Emergency appointments
Dental products
Laboratory fees
Third-party fees
Services provided outside Maxicare Dental
Any treatment not included in the approved treatment plan
Can the loyalty benefit be combined with other offers?
No. The 5% Loyalty Benefit cannot be used with any other offer, promotion, discount or payment arrangement unless Maxicare Dental confirms otherwise in writing.
